Billing Information:
- To enable us to serve you in this era of lowering reimbursements and increasing costs, we must request that all patients honor their payment responsibilities.
- All patients are required to pay, or make arrangements to pay, for services when services are received.
- Patients with insurance are responsible to pay any co-payments, co-insurance and/or deductibles that may be due.
- We accept cash, check, money order, and major credit and debit cards. Unpaid balances are subject to interest.
- Payment plans may be made through our various Account Coordinators or Collection Coordinator.
- If your bill remains unpaid after several attempts by us to collect, your account will be forwarded to a collection agency for further action.
Insurance Plans:
Medical Associates, P.C. participates with the following providers;
- Blue Cross/Blue Shield
- NewWest
- Allegiance - MSU
- Medicare
- Medicaid
As stated previously herein, you are responsible for your co-pays and/or deductibles at the time of service. If your amounts are unknown, you will be required to pay 20% of your charges at the time medical services are rendered.
Claims:
As a courtesy to you, we file all primary and secondary insurances.
If we do not participate with your insurance carrier you will be responsible for 100% of your charged services, regardless of your insurance company’s arbitrary determination of “usual and customary” fees.
Minor Patients:
The adult (parent or legal guardian) accompanying the minor is responsible for payment. The parent or legal guardian is also responsible for all medical charges incurred by an unaccompanied minor.
Prescriptions:
On prescription refills, we kindly ask that you first check with your pharmacist to see if you have any remaining refills available. If you need to request a prescription refill, we appreciate your efforts in giving us at least 24 hours to process your medicine request. This will ensure you receiving your medications in a timely, convenient manner.
For your convenience we now have a prescription refill request line available. You are welcome to call us at 587-5123, ask for the “prescription refill” line and provide us with your specific information.
Medical Records:
There is an up front nominal fee for your medical records requests.
If you need a copy of your medical record sent to another physician, attorney and/or insurance entity, please have them request the records directly. You will need to sign a Medical Release Form with them.
